Workshop Feedback Form Template
Feedback forms that measure what workshops should deliver: learning outcomes, skill development, and practical value.
Workshop feedback must measure more than satisfaction — it must measure learning. Did participants acquire the skill the workshop promised? Can they apply it in their work? Was the facilitator effective? Were the materials useful?
This template focuses on Kirkpatrick Level 1 (reaction) and Level 2 (learning) evaluation with questions designed to surface actionable improvement insights for the facilitator.
Designed for completion in the last 5 minutes of the workshop (not sent as a follow-up email, which reduces response rates dramatically).

Tips for Using This Template
Collect feedback in the room, not via email — response rates drop from 80% to 20% when sent post-event.
Ask the "before and after" skill rating question — the delta is a direct measure of learning impact.
Include one open-ended question: "What one thing would you change?" — it generates the most actionable feedback.
Share feedback results with the facilitator within a week to support their professional development.
