Workshop Evaluation Template
A multi-level evaluation framework that measures reaction, learning, behavior change, and business impact.
Workshop evaluation should go beyond "Did you like it?" to measure "Did you learn it?" and ultimately "Did you apply it?" The Kirkpatrick model provides four levels of evaluation, and this template addresses the first three.
Level 1 (Reaction) is collected immediately. Level 2 (Learning) is measured through pre/post skill assessments. Level 3 (Behavior) is assessed via a 30-60 day follow-up survey.
Designed for training departments and professional facilitators who need to demonstrate workshop ROI to stakeholders.

Tips for Using This Template
Collect Level 1 in the room. Send Level 2 pre-assessment during registration and post-assessment at the workshop end.
For Level 3, send a brief 5-question survey at 30 days. Keep it short — response rates drop for follow-up surveys.
Compare pre and post skill ratings to calculate a learning delta — this is your most compelling ROI metric.
Present evaluation results to stakeholders quarterly to justify continued training investment.
